Blepharoplasty/Eyelid Surgery– How It’s Done

The natural crease or fold when the eye opens or closes.To understand how an upper eye lift or blepharoplasty is done, let’s look at a patient’s left eye. In this photo the patient has closed her eye, and we can see that a natural line exists about 8mm from the lashes.

The lower edge of the incision during blepharoplasty.This line, marked by the blue arrows, represents the natural crease or fold when the eye opens and closes. This line, will serve as the lower edge for our incision during blepharoplasty surgery.

The upper edge of the incision during blepharoplasty.Next, we mark the upper edge of the incision. This is done by estimating how much excess skin needs to be removed during blepharoplasty surgery. This is shown by the green dots.

The extra skin removed during blepharoplasty surgery.By connecting the lower and upper edges of this design, we have created the area of extra skin that can be removed during blepharoplasty surgery. (This is highlighted by the yellow area). After the skin has been removed, the upper edge of the skin is sutured with tiny transparent suture to the lower edge; this preserves the natural eye fold and makes the incision virtually invisible.

For the lower eyelid lift, the incision can be done on the skin underneath the eye, or on the inside of the lid. The type of incision is planned according to each patient’s needs. Regardless of where the incision is placed, both approaches leave an undetectable incision.
