The Facelift Process

1. A personal, in-depth consultation

The first step for undergoing a facelift is a consultation with Dr. Siegel. The amount of information available online regarding various types of facelifts can be confusing, inaccurate, and overwhelming for most patients. During your visit, Dr. Siegel will discuss with you which options are best, which are just fads, and (most importantly) what is best suited for you. During this time, he will listen to your concerns, discuss your desired outcomes, and examine the unique contours and features of your face. He will then craft a personalized option to restore your appearance to the fresh, natural, youthful, look you once had or always wanted. Dr. Siegel can generate a three-dimensional facelift “before‑and‑after” image so you can know what results to expect. 2. Preparing for a facelift

Since each facelift surgery is different, the guidelines Dr. Siegel gives you to prepare for your procedure may vary. Other pre-operative safety precautions, like staying properly hydrated and not taking medications that could interfere with anesthesia or thin the blood – are standard for all surgeries. 3. The facelift procedure

Because your procedure is customized to you and your needs, the specific techniques Dr. Siegel uses for your facelift will depend on the current condition of your facial skin, the degree of sagging, drooping, jowls, creases, lines, wrinkles, excess fat, or loss of facial volume. The duration of a facelift can differ, but it typically lasts about 2 to 4 hours. Most often, Dr. Siegel will recommend the safest anesthesia options for each particular patient. Options include a combination of local anesthesia with intravenous sedation, and general anesthesia.

As each procedure is customized, the specific techniques Dr. Siegel uses for your facelift will depend on the current condition of your facial skin, the degree of sagging, drooping, jowls, creases, lines, wrinkles, excess fat, or loss of facial volume. The length of a facelift can differ, but it is typically about two to four hours. Most often, Dr. Siegel uses just twilights sedation, a combination of local anesthesia with intravenous sedation, rather than general anesthesia.

Older facelift techniques involved pulling the sagging skin, removing the excess, and repositioning it over the existing muscle structure, often resulting in an odd, over-stretched or “windswept” appearance. Dr. Siegel evolved the Vertical Facelift™ method after years of performing facelift surgeries. As gravity causes the facial tissues to descend, Dr. Siegel “re-suspends” them in a vertical, or “antigravity” direction.

Why a deep plane facelift is probably best for you

Dr. Siegel is one of relatively few facial surgeons (only about 5% at the moment) who perform deep plane facelifts for patients. We describe deep plane facelifts more on this page, but we can also describe them briefly here. It will make more sense if we first describe the main alternative: the traditional facelift.

Traditional facelifts or SMAS facelifts

There are several kinds of traditional facelifts: full facelift, mini facelift, lower facelift, weekend facelift, ponytail facelift, and S-type facelift (to name just a few). But they all have one thing in common: they rely on the traditional technique for facial rejuvenation, in which the skin is separated from the underlying fibromuscular layer of the face, known as the SMAS.

The SMAS is then pulled horizontally, or backwards, to reverse the signs of aging. This approach creates skin tension and leads to the distorted, pulled, “always looks surprised” appearance everyone fears.

Deep plane facelifts

The deep plane facelift, devised and refined only in the last decade or so, has become the gold standard. It allows for natural-looking results and longer-lasting results, and it requires much less recovery time. In a deep plane facelift the skin is not separated from the SMAS. Rather, the surgeon releases key ligament groups weighing the face down, around the cheeks, jaw, and neck and moves those tissues vertically.

That counteracts the pull of gravity and recreates the natural facial contours lost in the aging process. Because essentially the “cutting” happens at a deeper layer, the face does not look as though it is under constant tension, because it is not. The result is a face that has both definition and a relaxed look.

Volume replacement is another advantage of the deep plane facelift. By working under the muscle, it allows access to the fat compartments that have sagged along with the muscle. The skin, muscle and fat, can be repositioned as a single block, restoring the volume loss of the face, and a more youthful appearance.

Natural results

A deep plane facelift focuses mainly on the midface – the area of the face most affected by gravity. Correction of this area softens jowls and nasolabial folds (lines running from the nose to the outer chin). It also addresses the droopy fat pads of the midface – the cheeks that often look deflated or flattened because of aging.

Less bruising and swelling

Because the skin has not been separated from the underlying tissues and there is no tension placed on it, the edges where incisions have been made can heal easily and with little scarring.

It can be done as a “ponytail” facelift.

The deep-plane technique, coupled with masterfully placed incisions, allow Dr. Siegel’s patients to still wear their hair in a ponytail, without worrying about visible telltale signs of facelift surgery.

A deep plane facelift is a great option even for men

The incisions are different from those in a female facelift. Because of the facial hair associated with men, incisions need to be designed differently, so as to camouflage the final appearance of the incisions. Though fillers may provide an option for volume loss as we age, fillers fill: they do not lift saggy facial features. A deep plane facelift sounds perfect. What’s the downside?


The cost: Only about 5% of facial plastic surgeons have the technical expertise and medical experience to perform a successful deep plane facelift, and that is reflected in the price tag. A deep-plane facelift can cost almost double of a traditional facelift, depending on where you go. But in the right hands, it yields uniquely natural-looking results with minimal downtime, and results that lost about twice as long as those of a traditional facelift.


Do you still do traditional facelifts?


Yes. Every patient is different. Despite all the advantages of a deep plane facelift, sometimes one simply is not necessary to achieve a patient’s best look. Dr. Siegel will look at your unique face and discuss your unique needs and goals with you.


What age range is normal to consider a facelift?


Usually 40+, though everyone’s situation is different. Most facelifts are performed on patients over 40 years old who notice facial drooping and who want a refreshed look. The most common age for a facelift is between the ages of 50 and 60. The right age for you to have a facelift will depend on your skin quality, health, and your personal aesthetic vision.


How long do the results of a facelift last?


Each person is different, so is no one answer to this question. When performed properly with advanced techniques, like those Dr. Siegel uses, facelift results can last up to 15 years or even longer.
