What Is Ultrasonic Rhinoplasty?

In traditional rhinoplasty, surgeons shape the nasal bones with mechanical instruments such as mallets, saws, chisels, and rasps. Narrowing or straightening the nose often requires “breaking” the bone. These tools can make it harder to control exactly how the bone is shaped and can affect the tissues around it.

Ultrasonic rhinoplasty uses a small handpiece that creates gentle vibrations absorbed by the bone. The vibrations are focused where they are needed, leaving the surrounding soft tissue largely untouched. This gives Dr. Siegel more control and allows him to make more precise changes while causing less trauma to the tissues around the nose. This can greatly reduce the bruising around the eyes and swelling often seen after traditional rhinoplasty.

What Are the Benefits of Ultrasonic Rhinoplasty?

This keeps the cartilage portion of the bridge intact and helps maintain the smooth, natural lines of your nose, called the dorsal aesthetic lines, instead of cutting the bridge apart and rebuilding it.

Is Recovery Shorter After Ultrasonic Rhinoplasty?

For most patients, yes. Ultrasonic rhinoplasty helps avoid the bruising and swelling around the eyes that can happen after traditional rhinoplasty by protecting the nearby tissues and blood vessels.

In traditional rhinoplasty, packing is often placed inside the nose to help control bleeding. With ultrasonic rhinoplasty, the blood vessels are better protected, so packing is usually not needed. That means a more comfortable recovery without packing inside your nose. You may still feel a little stuffy, like you have a cold, for the first 48–72 hours. 

With ultrasonic rhinoplasty, there is usually much less swelling, so you can begin to see the shape of your new nose when your external splint is removed at 7–10 days. Traditional rhinoplasty can leave more swelling that may hide your results for months. Your nose will continue to heal, settle, and look more refined as the weeks go by. Wait at least three weeks before vigorous exercise, and read the ultrasonic rhinoplasty aftercare instructions ahead of time so you know what to expect throughout the process.

Do Most Nose Surgeons Perform Ultrasonic Rhinoplasty?

Not many do yet. Dr. Siegel was the first facial plastic surgeon in Houston to incorporate ultrasonic rhinoplasty—and now you don’t have to book an international flight to get it.

Ultrasonic rhinoplasty was developed in Paris, France, by Dr. Olivier Gerbault and later became more widely known in the United States through Dr. Aaron Kosins. The technique gained wider attention after Dr. Gerbault and Dr. Kosins, two of the three authors of the 2016 Aesthetic Surgery Journal paper on piezoelectric instruments in rhinoplasty.

The technique also became widely used in Turkey, where Dr. Siegel trained in Turkish rhinoplasty techniques. His training in ultrasonic rhinoplasty began in 2021, when he studied directly with both Dr. Gerbault and Dr. Kosins. This specialized training gave him experience with the technique from two of the surgeons who helped establish its use in modern rhinoplasty.

What Can Ultrasonic Rhinoplasty Correct?

Ultrasonic instruments are used wherever your nasal bones need to change.

• A bump on the bridge — The bony part of a hump is smoothed down without traditional bone breaking or rasping (similar to a nail file)
• A wide or strong bridge — Nasal bones that are too wide or too strong for your face can be narrowed and refined.
• A crooked nose or uneven bones — Bony asymmetry from birth or from an old injury can be evened out as part of crooked nose surgery.
• Breathing — A deviated septum can be corrected in the same operation, and it doesn’t typically add to your recovery time.

Changes to the tip and nostrils involve cartilage and soft tissue, so a bifid tip, a change in nostril shape, or the softly lifted tip of a Barbie nose can be addressed with other techniques during the same surgery.

Is Ultrasonic Rhinoplasty Right for Every Patient?

Nearly every rhinoplasty that involves any adjustment to the nasal bones can be done ultrasonically, so the question is usually more about what your nose needs. That includes ethnic rhinoplasty and Asian rhinoplasty, where bone work is often combined with building structure, and revision rhinoplasty after an earlier surgery.

Ultrasonic instruments change how your bone is shaped, not the rest of the operation, so it’s still surgery with a real recovery, yet with much less trauma and swelling.

At your consultation, Dr. Siegel will listen to what bothers you, examine your nose and your breathing, and use computer imaging to show you what’s possible. The images are a planning and communication tool rather than an exact preview. If your bones don’t need to change, he’ll tell you.

What happens on the day of surgery?

You’ll have general anesthesia with a board-certified anesthesiologist in our private surgical suite in Houston. Most cases take one to three hours, and you go home the same day with someone to drive you.

Is ultrasonic rhinoplasty open or closed, and will I have a scar?

Most of Dr. Siegel’s rhinoplasties are closed (scarless), with every incision inside your nose, and he chooses the approach for your anatomy and your goals. If an open approach suits your nose better, the only external incision is a small one between your nostrils, and it’s typically very difficult to detect once it heals.
